Default What's wrong with this cat?

The cat in question is my neighbor's; I occasionally 'hang out' with him/her because they leave their garage open, where it lounges. There seems to be some lumps in various places and it breaths odd. The lumps are behind the head and around its collarbone; initially I thought it might just be fat deposits, but parts of the lumps feel hard. Its breathing makes a sort of ripple effect if you look at its stomach.

I know he/she didn't used to look like this; it was much healthier looking a year ago. Should I talk to my neighbors about this and/or call my local animal shelter?

I've cross-posted this to a few feline forums in hopes of getting additional advice; I really want to help this cat and I'm questioning the owner(s) about him/her tomorrow. Is there anything else I can do though?
