Default Cat pooping outside litter box

Hi
This is my first post. I have 2 cats that I am sure I will be asking ??? about, however this is about my sisters cat. Let me list the situation as briefly as possible:
Cat is a neutered male, about 7 or 8 from a rescue and she got him at about age 2. She is a teacher (so you know her schedule). She is married, has a male pug that they seem to get along well with eachother. The problem is that he went from sporadically pooping outside his litter box to ONLY pooping outside the litter box. He is a declawed indoor cat.
She has tried: moving the litter box, changing food, antidepressants, confining him, all kinds of blood tests and other tests....all over the course of the past 2 years. She is now married, planning to have kids and loves the cat. He is otherwise, totally fine.
Anyway, nothing has worked and they are getting really tired of it all and feel like they have exhausted all of their and their vet's ideas. It is getting critical. Any help you can give would be great.
Let's say he is mad about the dog....well, what do you do realistically...although my guess is it is something else....Thanks!

Have you tried changing to a different type of cat litter. Also, when he does poop is it usually on the rug or what. You might try putting down a newspaper where he usuallly poops. Also it is vital that you get rid of the smell as he will keep going back to that. My cat will poop just outside the litter box if he thinks it isn't clean enough.....and by that, I mean sifted EVERY day. Talk about super fastidious.

She has tried different litter, different food, different foods, cleaning it...yes! I will have her try to put newspaper around it but anymore suggestions will be welcomed! thanks!

Does he go in the same place every time? They can get awful picky about having a clean box. Ours would always race to see who could go first after a cleaning. It might not be up to his cleanliness standards.

I agree with others about cleaning daily. Our cat has gone on the floor a few times. I can relate that to times when sifting was late. If he skips a day then goes twice in a day this can lead to poop on the floor.

On a related issue, I am attempting to toilet train our cat. I have had a bowl in the bowl for several weeks. This was the weekend to cut a hole in the bowl but kitty has a severe resparatory(sp) infection. The point is that this bowl is cleaned after every use just to keep poop off the floor.
