Bronchial Asthma has become an epidemic that causes 4 to 5 thousands deaths annually in the United States. Were patients to have healthcare access and be properly treated, deaths associated with asthma would be infrequent and hospitalizations would be rare.While a medical school at UCLA in the 1980's, the pediatric protocol for patients with acute asthma was to administer Epinephrine HCl (aqueous (Adrenalin), 1:1000 at .01 ml/kg/dose to a maximal dose of .3 ml SC every 20 minutes. Epinephrine has a very short half-life. If not improved, patients were usually hospitalized. Why had the UCLA professors not read about Che and his asthma?I researched that data on my own (as I do all of my work), and I learned that Che Guevara ha already been taking lots of Prednisone (corticosteroids) in the 1960's for his asthma on an intermittent basis. Though he was a severe asthmatic, Che managed to cause us a lot of trouble in the jungles of Cuba and South America. Our bodies make cortisone, at a physiologic dose of at least 0.5-0.75 mg/kg/24 h and life cannot exist without it.Therefore, the most important medicine in acute asthma is a corticosteroid like Prednisone that is dosed at 0.5 mg-2 mg/kg/24h up to 40 mg orally for 3-5 days per month. The most important drugs for asthmatics are the inhaled cortisone as in Pulmocort®, QVAR® and short acting Albuterol inhalers. I do not prescribe Advair® since it has been associated with 'asthma-related" deaths and is expensive.In order to rescue patients, I have had to quickly inject them Epinephrine (Subcutaneously) at the appropriate dose, Dexamethasone, and put them on albuterol nebulizer. I have saved many patients and rarely had to be hospitalized. The bronchioconstriction can be so severe that Epinephrine may still be essential in order to rescue a patient and allow the albuterol to reach the targeted receptors (beta-2).Bronchial asthma is commonly misdiagnosed as just a "cold." The history is of utmost importance in diagnosing asthma. Asthma is a nocturnal condition and children may seem well during the day. Clinically, I ascultate such patients while crying since this creates bronchial turbulence, and I may then hear the wheezing or the crush sound. The Lomeli Sign is simply used to detect bronchial inflammation and hidden wheezing. I ask the patients to cough as I ascultate. If there is inflammation, I often hear the crushing sound as heard on this video. A mother once said to her daughter: "Don't cry because you'll vomit." The latter is typical of asthmatics.Congestion may actually help prevent the antigen (allergen) from reaching the pulmonary passages. I will cover this theoretical topic in the near future. Doctors must examine family members for a history of asthma and for signs of allergy: Pterygium, hay fever nose mark and nasal congestion that lasts more than 12 days. Most children that present to doctors with a night cough and "phlegm" have bronchial asthma. Bronchial asthma usually resolves in 3 to 5 years as the body develops specific defenses. The patients that I have studied with a history of bronchial asthma have normal pulmonary function tests as adults.Dimetapp, Triaminic, Pediacare, Robitussin and similar "cold products" do not have a curative benefit. Mucinex® contains the run-of-the mill expectorant Guaifenesin and has no therapeutic value. The maker of Mucinex® (Adams) promotes the product as "Mucus in. Mucus out™". I am surprised that our FDA has not intervened in regard to this product. Marketing can make a nonsense product seem of value.Promethazine is contraindicated in patients with asthma.
